I’ve kept the free printable questionnaire simple so kids of all ages can join in the fun. These are fill-in-the-blanks that kids can write down their own answers. We all know young kids say the most hilarious things, so you know moms will surely get a good laugh while reading them.

The free printable Mother’s Day questionnaire includes simple fill-in-the-blank like:

There are fifteen Mother’s Day interview questions on this printable, and I’ve included one for Grandma, too. There’s even a special spot at the bottom where kids can write three things they love most about her. Doing this for moms is a fun gift idea that kids can do, and it makes a great keepsake. Kids grow up so fast that having something like this to look back on their kids’ answers from the previous years will be something everyone will cherish, not just moms. 

More Ways to Use this Mother’s Day Printable

If you don’t have a printer at home, another alternative would be to do a Mother’s Day video interview. Not only are you still participating in this, but you’ll still be able to have the perfect gift for Mom. The only difference is that this will be on video instead of paper.

A video interview would be a great way for little kids who might not know how to write yet. Just record the child’s responses to the questionnaire and then send the video to mom or grandma.
