I’ve kept the free printable questionnaire simple. This is a fill-in-the-blank activity where kids can write their own answers in the blank spaces. Kids say the most hilarious things, so you know Dad’s will get a good laugh while reading these. 

The free printable Father’s Day questionnaire has simple questions like:

  • His favorite color is… 
  • His favorite food is… 
  • He always says… 
  • My Dad is really good at… 
  • He is as smart as… 
  • My Dad’s job is… 
  • His favorite thing to do is…
  • He loves to spend time…

There are fifteen Father’s Day blank questions on this printable, and I’ve included one for Grandpa, too. There’s even a special spot at the bottom where kids can write three things they love most about him. 

More Ways to Use this Father’s Day Printable

If you don’t have a printer at home, another alternative would be to do a Father’s Day video interview. Not only are you still participating in this, but you’ll still have the perfect gift for Dad. The only difference is that this will be on video instead of paper.

A video interview would be a great way for little kids who might not know how to write yet. Just record the child’s responses to the questionnaire and then send the video to Dad or Grandpa.
